Patient Care Assistant III (PCA III)

Nexus Children’s Hospital – The Woodlands is the nation’s only inpatient program dedicated to children, adolescents, and young adults with Prader-Willi Syndrome (PWS) and childhood obesity. The Patient Care Assistant III (PCA III) is a senior-level caregiver and operational leader within our patient care team. Working under the supervision of licensed nursing staff, this role combines advanced clinical and behavioral support with leadership responsibilities, including supervision of PCA staff, coordination of care tasks, and quality management. The PCA III plays a vital role in mentoring team members, ensuring patient safety, and supporting a culture of excellence.

• Leadership & Service

  • Supervise PCA I & II team members and coordinate daily workflow.

  • Champion Nexus’ Mission, Vision, and Values through compassionate care and ethical conduct.

  • Collaborate across departments to ensure seamless service delivery.

• Clinical Excellence

  • Provide advanced support in wound care, behavioral de-escalation, and critical interventions.

  • Act as liaison between PCA team and nursing staff.

  • Serve as lead responder during behavioral or safety escalations.

• Patient Advocacy

  • Maintain a safe, therapeutic environment and ensure patient dignity.

  • Support patients through engagement strategies, crisis intervention, and trauma-informed care.

  • Facilitate patient-centered decision-making and family involvement.

• Quality & Compliance

  • Lead or co-lead quality improvement initiatives and process audits.

  • Ensure compliance with HIPAA, accreditation standards, and infection control protocols.

  • Maintain accuracy and consistency in clinical documentation.

• Education & Mentorship

  • Precept and mentor PCA I, PCA II, and new hires.

  • Conduct training, educational in-services, and performance feedback.

  • Stay current with industry standards, certifications, and best practices.

• Resource Management

  • Promote stewardship of hospital resources.

  • Manage supply inventory and recommend cost-saving initiatives.

• Education : High school diploma or equivalent required; CNA program or advanced patient care training preferred.

• Experience :

  • Minimum 3 years in direct patient care, including 1 year in a leadership or supervisory capacity.

  • Experience supporting patients with complex medical, psychiatric, or neurobehavioral needs.

• Skills :

  • Proficient in advanced patient care tasks (wound care, EHR, POCT, Q15 checks).

  • Strong leadership, mentorship, and conflict-resolution skills.

  • Knowledge of trauma-informed care and crisis intervention.

• Licensure/Certification :

  • Current BLS certification required.

  • CNA certification preferred.

  • De-escalation training required within 30 days of hire (trainer certification preferred).
